The primary responsibility of ECFMG is to assess the academic qualifications of international medical graduates, ensuring their educational level is comparable to that of American Medical Graduates (AMGs). Its core functions include:
Qualification Verification: Verifying the legitimacy of medical schools worldwide and the authenticity of applicants’ degrees.
USMLE Administration: Partnering with the Federation of State Medical Boards (FSMB), ECFMG administers and manages the first two steps of the United States Medical Licensing Examination (USMLE).
Visa Sponsorship: ECFMG is the only institution authorized by the U.S. Department of State to sponsor J-1 visas for IMGs pursuing Graduate Medical Education (GME) in the United States.
2. Core Elements of the Certification Process
To obtain ECFMG certification, applicants must complete a series of complex verification processes, fully integrated into the MyIntealth digital platform by 2026:
Medical Science Assessment: Passing USMLE Step 1 (Basic Medical Sciences) and USMLE Step 2 CK (Clinical Knowledge).
Clinical and Communication Skills Verification: Following the elimination of Step 2 CS, IMGs must demonstrate their clinical practice and communication abilities through designated pathways (such as holding a national license or passing the Mini-CEX assessment) and the OET Medicine (Occupational English Test).
Initial Certificate Verification: ECFMG will directly contact the graduate’s alma mater to verify the authenticity of their medical degree.
3. Major Changes After 2024
Since 2024, ECFMG has implemented a **“Institutional Accreditation Policy”. The medical education accreditation body in the applicant’s medical school must be recognized by the World Federation for Medical Education (WFME). This policy has significantly promoted the standardization of global medical education and placed higher background requirements on IMGs.
4. Professional Value and Significance
Obtaining ECFMG accreditation not only means that the applicant possesses the theoretical foundation to practice medicine in the United States, but it is also a prerequisite for entering the NRMP Match (Residency Matching System).
